What is a QSEHRA?
Qualified Small Employer Health Reimbursement Arrangement
Your tool for improving benefits at a lower cost
A Qualified Small Employer Health Reimbursement Arrangement (QSEHRA) is an IRS-approved way for small businesses with fewer than 50 employees to reimburse staff for out-of-pocket health expenses — completely tax-free for both the employer and employee. Instead of offering a traditional group health plan (which can be expensive and complex), you set a monthly reimbursement allowance. Employees pay for qualifying expenses, submit proof, and get reimbursed. You take the deduction; they pay no income tax on it.
2025 contribution limits: Up to $6,150/year for a single employee (~$512/month), or up to $12,450/year for employees with a family (~$1,037/month).
How to qualify
Can a sauna membership qualify?
Yes — with one important nuance. Under IRS Publication 502, wellness and fitness expenses qualify for reimbursement when they are recommended by a physician to treat a specific diagnosed condition, such as high blood pressure, chronic pain, cardiovascular disease, or obesity. Many of our members already use sauna and cold therapy as part of a care plan. If your employee has a qualifying condition, their doctor can provide a simple letter of medical necessity — and their membership becomes a reimbursable health expense.
Not every employee will qualify under QSEHRA – but many will. For employees who don’t have a qualifying condition, some employers pair their QSEHRA with a separate taxable wellness stipend that covers memberships for everyone.
